If you are a new patient to our clinic, please bring a completed and signed copy of the following form and bring it to your first appointment.

What to bring to your appointment

• Please bring your Medicare card
• Your referral letter
• Your glasses (if any) and/or any contact lens containers
• A list of current medications and allergies is helpful

 

How long will the appointment take

Please allow at least 2 hours for the appointment, especially if it is an initial consultation.

 

Fees

A copy of our fee schedule will be provided to you at your first appointment. New patients can expect to pay around $200-$500 for their initial consultation depending on the number of tests required, and patients with a Medicare card may be eligible for a rebate.

 

How to get to our clinic

Our clinic is located in the East Wing Consulting Suites of Epworth Eastern Hospital, in the Box Hill Medical Precinct. The entrance is located at 25 Nelson Road, opposite the Box Hill RSL. We are easily accessible by car and various forms of public transport.

Car Parking

A patient ‘drop-off’ area is available immediately out front of the East Wing Entrance on Nelson Rd. This is the fastest way to access the lifts up to level 12 to our suite.

Driving yourself to an appointment is generally not recommended as we often will need to dilate your pupils to complete the full eye examination.

Car parking is available onsite at the Epworth Eastern Hospital underground car park via the Arnold Street Entrance.  The cost for parking here is set by Epworth Eastern and is approximately $18 per hour.  

Metered parking is also available on the streets surrounding the hospital (2 hour maximum) and further away there is free non-metered parking for 2 hours (Check signs).

Alternatively, patients can park at Box Hill Central Parking at a rate of $1 per hour up until 4 hours and can walk 5 minutes via Nelson Road to the hospital.

 

Tram and train

Our clinic is a 4 minute walk from Stop 57 (Nelson Rd) on Tram Route 109 and within 10 minutes of Box Hill Train Station on the Belgrave/Lilydale Line.
